2020-2021 Sports Registration/Sports Qualifying Physical Exam Forms  Update 07.30.2020
Registration for Fall SAHS Athletics is scheduled* to open on Friday, July 31, 2020.  
Fall Sports are currently scheduled* to start on Monday, August 17
*Dates are subject to change based on upcoming decisions/announcements made by the Governor (scheduled for  Thursday, July 30, 2020), Stillwater Area Public Schools and the Minnesota State High School League.    
Please continue to check this website throughout July and August for more specific information regarding our new online registration system and sports specific information.

Information from the MSHSL regarding the fall season is expected to be announced on August 5 or 6, 2020 and will be posted on our website.

Before attempting to Register for SAHS Athletic or Independent Provider Team:
  • SAHS Students must have a current Sports Qualifying Physical Exam Form on file in the SAHS Activities Office (not the Nurse's Office).   (Immunization records are not required in the Activities Office, but must be submitted to the appropriate Nurse's Office.)
  • Middle School students must have a current Sports Qualifying Physical Exam Form on file in the SAHS Activities Office (copies on file in the Nurses' Office at SMS or OMS do not meet this requirement).  
Check the Date of Your SQPE
  • Parent or Guardian Log In to Skyward
  • Select Report Cards/Portfolio
  • Select Sports Physical to see the date of the most recent exam on file.  
  • Exams expire 3 years from the date of the exam.
Please Note:
  • It is the responsibility of the student athlete and parent/guardian to provide a copy directly to the Activities Office.   
  • Nurses' Offices at SAHS, SMS and OMS are not open during the fall registration period or on day one of fall sports.  Access to exam forms you may have on file in these offices will not be available to access.
  • Copies of "Well Child Checks" do not meet the SQPE requirements.
  • Exams are good for 3 years from the date of the exam.  The SQPE may not expire during the season for which you are registering. You will be unable to register if your SQPE has expired - no exceptions.
  • Scan your completed exam form and submit directly to Jacki Delahunt:   
  • Please allow 48 hours for the exam to be entered in to the system before attempting registration.
