Registration for all activities and athletics are done electronically via SmartSchoolK12.
You will need your student's ID number, which can be found in Skyward.
Both the student and a parent/guardian should be present when registering.
Electronic signatures are required from both the parent/guardian and student.
Student-athletes must have a current Minnesota State High School League (MSHSL) Sports Qualifying Physical Exam (SQPE) on file in the SAHS Activities Office. Note: a Well Child Check is not considered a SQPE.
The SQPE Form expires 3 years from the date of the exam.
You will not be able to register for an activity if the expiration date occurs during the season for which you are registering.
The physician/clinic representative must complete, sign and date the form.
Submit a copy of your SQPE Form to Jacki Delahunt in the SAHS Activities Office.
Allow at least 48 hours from the time you submit your SQPE form, for it to be updated in Skyward and SmartSchoolK12.
Keep a copy of your SQPE for your records.
